Your medical records are confidential.
The practice is registered and complies with the Data Protection Act 2018. Any request for access to notes by a patient, a patient’s representative or an outside body will be dealt with in accordance with the Act.
You have the right to ask for access to, or obtain a copy of, your medical record ie. the information we hold about you. Please contact the Practice for further information. We will require a signed consent request (form available in the Library section of the website).
No information about you will be given to third parties without your consent, with the exception of information given to another doctor or health professional for the purposes of your medical care. Please see our Privacy Notice, available on the website under Patient Information, for full details of how we process and share information about you.
It is our Policy not to release your GP record to anyone but yourself.
Reports from your medical records may be requested by insurance companies, employers etc. Reports will not be prepared without your permission in writing and you will be asked to state whether or not you wish to see the report before it is sent.
