Fit notes

You only need a doctor’s note if you are unable to work and are ill for longer than 7calendar days.

Your employer will provide you with an SC2 self-certificate form for shorter periods of illness.

If you cannot get an SC2 form from your employer you can find them on the Government website on the following link;

We do not have copies of the Sc2 form in the practice to hand out

Please read the following information before requesting a sick note

We cannot accept requests for fit notes over the telephone. All requests, where possible, should be submitted in the form of an online e-Consultation through out website. You will find the link at the bottom of this page;

If you do not have access to the internet, cannot access a community internet capable facility or do not have a family member or friend able to assist you then we will accept phone calls.

For new fit note requests

Before a new fit note can be issued a DR will first want to have a consultation with you to determine the requirement for the fit note and potentially discuss treatment.

If you need a private sick certificate you will be charged a £10 fee, which is in line with BMA approved national guidelines and is displayed in the surgery.
